It happens, even with factory loads back in the 70's and 80's. We had lots of them brought to my dad's gun shop for me to drill out. We had pilot sleeves built in different lengths and calibers with long drills do we didn't damage the riffling on the barrels. I stacked several in an old .380 semiautomatic before I noticed it, all factory loads. Scarred the heck out of me once I figured out what had happened. That was the last time I wore ear plugs and ear muffs while shooting. I couldn't hear a dang thing and thought the combination was doing a great job.
Never saw one sticking out the end of the barrel though.

Thats why I will never shoot any handload that I didnt personally load. Seen too many careless mistakes, alot of times when buddy thought it’d be a good idea to mix reloading and alcohol.

Good oversight. Handload? Yep. My dad’s. Sounded like primer-only. Wow!! To get a bullet that far down the barrel, there had to be some powder in it. The primer doesn't have enough juice to get it much further than past the start of the rifling, especially in a revolver where you have the cylinder gap. He most likely under charged the case.
