When I watched him last week, his tongue was always out as well. The problem is own both sides but only have pics of his right side. Any guesses?

Looks like a bad tick infestation to me. Looks like he has been scraping against trees/brush to get them off. If he doesn't get them off he is a goner.

It takes ticks and fleas several weeks/months to build up to the point you see in the photo. If you look closely the health of the deer is less than normal following rut.
