Noticed this doe on the camera with this reddish sack hanging from her belly. Any ideas on what's wrong with her?

looks like a hernia, you can almost make out the bowels.

Seen it on an occassional doe when they have fawns. Could just be an udder that has broken down with age. I have seen a buck with a hernia never a doe in all my years but it is still possible.

If thats a hernia, will it ever heal or go back to normal? That sucks to see that.

This is very true, a hernia is caused by the abdominal wall weakening and letting the bowels pertrude through the abdominal muscle. That being said we as humans are not putting the same pressure on our abdomen like a deer would. So we can push a small hernia(1-2cm) back in but a large one(2+cm) needs surgical repair.
I dont believe (if it is a hernia) this will go back to normal on its own.
If it is mastitis, it might get better over time, if she has weaned her fawn off her milk.
